Family Support Services Program Program de Servicios is a program designed to keep families together. All people with developmental disabilities have the right to live in a stable home; building relationships with parents, siblings, and friends as they enrich each other's lives.Children and adults with developmental disabilities have the best opportunity to grow and develop within a loving family environment. With the help of Family Support Services, families can purchase necessary equipment and services thus easing some heavy expenses while increasing the self-sufficiency of a family member with disabilities. The Family Support Services Program is a state-funded financial reimbursement program helping eligible families to keep family members with developmental disabilities living at home by assisting with specific expenses. The Family Support Services Program makes co-payments with the family to approved providers. The amount of family assistance is determined by a scale based on the federally taxable income of the person(s) who provide for and live with the individual with the disability. The Ohio legislature determines a maximum amount of assistance that is available for each county during a program year which runs from July 1 to June 30. The available funds, unfortunately, rarely meets all requests which are handled on a first-come, first-served basis until funding is exhausted.Families are eligible for Family Support Services if their household includes a child or adult with a developmental disability. The person must be determined eligible by Butler, Clermont, or Hamilton County Boards of Developmental Disabilities Services, depending on the legal residence of the person. If they are determined eligible but are not receiving services from the County Board of DD, a family may still be eligible for this program. The disability must have become apparent before the age of 22 with the likelihood of its continuing indefinitely. For young children, there are more liberal state eligibility definitions because early intervention can lessen the effects of disabilities on a child's future. It is important to note that Family Support Services must determine a family's eligibility and approve their services before any expenses can be processed. Expenses that occur before a family becomes involved with Family Support Services are not covered. The Expenses that may be covered:•· Respite Care (both in and out of home)•· Adaptive Equipment•· Special Diets•· Parent and Sibling Education, Training, & Counseling•· Home Modifications and Other Approved ServicesHamilton County Developmental Disabilities Intake Number 794-3300Butler County Developmental Disabilities Intake Number 867-5962Clermont County Developmental Disabilities Intake Number 732-7000Contact Sandy Schutte at The Arc (513) 821 2113 ext. 109Respite Provider Application 2009Respite Waiver for the FamilyVerification of need formTaxable Income CertificateCLERMONT COUNTY REQUEST FOR VOUCHERS 1ST QUARTER 2010 HAMILTON COUNTY REQUEST FOR VOUCHERS 1ST QUARTER 2010 Computers 4 Home Cincinnati Computer Cooperative |
